The relationship between vaccines and autoimmune diseases has been the subject of numerous studies, the results of which show that in some cases there may be a worsening of the symptoms of these conditions. However, the possible occurrence of worsening reactions does not justify choosing not to undergo vaccination, as clearly stated by EULAR (European Alliance of Associations for Rheumatology) in the recommendations for vaccination in adult patients with autoimmune diseases, published in 2019.

We can confirm that recently, at the Dermacademy Institute, we have examined around 200 patients vaccinated against Covid-19, and so far we have not observed any worsening of the condition in any case.

To date, experience with mRNA-based vaccines in patients with autoimmune diseases is still too limited to draw conclusions regarding their safety and effectiveness, as explained in an article published on the website of the Mario Negri Institute for Pharmacological Research. Although there are no “direct data on the effects of vaccination in these individuals,” it is reasonable to assume that, when weighing risks and benefits, vaccination also offers advantages for these patients.

The recommendation of the Mario Negri Institute is to allow Covid-19 vaccination for immunocompromised patients, always evaluating each individual situation together with the treating physician, who is the professional most familiar with the patient’s health condition.

In the absence of specific data deriving from clinical studies conducted on subjects affected by autoimmune diseases, it is useful to refer to the indications provided by the specialists of the Mario Negri Institute, published on the official website.

Can Covid-19 vaccines safely activate the immune system?

According to the major scientific societies, the vaccines currently used in the Ministry of Health vaccination campaign are mRNA vaccines and therefore do not contain “live attenuated” viruses, but only fragments of this molecule. This reduces concerns regarding their safety when administered to immunocompromised individuals. As with anything, however, patients belonging to this category should be carefully monitored when undergoing vaccination.

Another aspect must also be considered. Many autoimmune or inflammatory diseases are characterized by phases of flare-ups of symptoms and signs, during which therapies are generally intensified or resumed. Although there are still no data deriving from clinical studies, specialists consider it inappropriate to vaccinate patients during periods in which the disease is manifesting again in a more acute phase: it is therefore necessary to wait for improvement.

Should patients undergoing treatment with anti-inflammatory and immunosuppressive drugs suspend therapy before vaccination, or can they continue it?

There is no definitive answer to this question either, because there is no experience deriving from clinical studies conducted on patients with autoimmune or inflammatory diseases.

However, according to experts, the administration of immunosuppressive drugs at the same time as vaccination could reduce the response to the vaccine. As a precaution, it is therefore suggested to suspend or postpone therapy if doing so does not compromise the patient’s condition.

In any case, this assessment is the responsibility of the treating physician. However, it remains difficult to establish how long the suspension should last.

In recent days, many patients have been asking us for information regarding contraindications and possible issues related to the administration of the Covid-19 vaccine for individuals affected by vitiligo.
